The Pathways HUB is a community effort to coordinate care and create closed-loop referrals for vulnerable people in our County. The HUB will double as the Coordinated Entry System (CES) for those experiencing homelessness. It will additionally serve those with behavioral health needs. The goal is to have more integrated systems of care between health, behavioral health, and homeless services.
